Home Care Coordination Program

Our goal is to increase our clients quality of care by focusing on all aspects of their wellness and quality of life. This can be done with a combination of our experienced Care Coordinators as well as community resources.
Allcare Home Care Coordinators ensure the delivery of services in a timely manner to meet the goal of continuity of care in the home. This sometimes involves organization of community resources.

Services Include:

On-site client assessment prior to discharge from facility. This service ensures a smooth transition from hospital/skilled nursing facility to the home.
Home Care: Personal care/Caregiver services; Bathing, Grooming, Dressing, Light House Keeping, Medication Reminders, Respite Care, ADL’s and Transportation.
Coordination of home health services including Nursing, Physical, Occupational and Speech Therapies; Medical Social Workers and Home Health Aides.

Participation in Rehabilitation team/family case conferences
.Assessment for appropriate intervention of disciplines, medical equipment and assist in coordination of appropriate equipment/supplies for home delivery.
Coordination of delivery of IV/Enteral Feeding Supplies.
Verification of insurance coverage.
Education to clients, families and caregivers on the disease process.
